Travel Reimbursement

If you drive yourself or arrange your own transportation to a Medicaid-covered medical appointment, you may be eligible for mileage reimbursement.

$0.725per mile

How to Apply

  1. 1Complete the Mileage Reimbursement Form (available above in Downloadable Resources).
  2. 2Attach proof of your medical appointment (appointment confirmation or provider signature).
  3. 3Include your Medicaid ID number and date of birth on the form.
  4. 4Mail or fax the completed form to the address listed on the document.
  5. 5Allow 2-4 weeks for processing after we receive your form.

Required Documentation

  • Completed Mileage Reimbursement Form
  • Proof of appointment (provider signature, appointment slip, or letter)
  • Valid Medicaid ID
  • Vehicle insurance and registration (if driving yourself)

Community Advisory Board

The Community Advisory Board (CAB) provides a platform for Medicaid members, caregivers, and community stakeholders to share feedback and help improve NEMT services in Colorado.

Board members advise on service quality, accessibility improvements, and member satisfaction initiatives. Your voice helps shape the future of medical transportation in your community.

Meeting Schedule

The CAB meets quarterly (January, April, July, and October) on the second Wednesday of the month at 5:30 PM. Meetings are held in person with a virtual attendance option.
