Gas Reimbursement Program
A friend, neighbor, or relative can be reimbursed for transporting you to Medicaid-covered medical appointments. You must call MediDrive before the trip to obtain an authorized trip number.
4-6weeks to payment
How It Works
- 1Call MediDrive BEFORE the trip to get an authorized trip number.
- 2Have your friend, neighbor, or relative transport you to the appointment.
- 3Get the Gas Reimbursement Trip Log signed by the physician’s office.
- 4Mail the completed trip log to the MediDrive Claims Department.
- 5Receive payment within 4 to 6 weeks after receipt of the completed log.
Required Documentation
- Pre-authorized trip number (obtained by calling before the trip)
- Completed Gas Reimbursement Trip Log
- Physician’s office signature on the trip log
- Valid Medicaid ID number
- Payee’s name and mailing address for reimbursement
